About ten years after the first hype, natural orifice translumenal endoscopic surgery (NOTES) has become part of endoscopic routine. Transmural necrosectomy, for example, is an important technique among minimal invasive treatment options for acute necrotizing pancreatitis. In Japan and China, “peroral endoscopic myotomy“ (POEM), “submucosal tunneling endoscopic resection” (STER) and “full thickness endoscopic resection“ (FTER) as newer NOTES-methods are standard techniques at high-end endoscopic centers meanwhile. The high patient volume due to the big population is an important factor for innovation that helped to develop and refine these techniques. Recently, G-POEM („G“ stands for „gastric“) is available for myotomy of the pylorus in patients with refractory gastroparesis. However, its therapeutic role has not yet been defined. Anyway, endoscopy has been enriched by NOTES which will bring up even more techniques in the future for sure. Japan and China will play a key role in this development.

Kurzfassung: Etwa zehn Jahre nach dem ersten Hype ist die „Natural orifice translumenal endoscopic surgery“ (NOTES) im endoskopischen Alltag angekommen. Als transmurale Nekrosektomie stellt sie eine wichtige Säule in der minimal invasiven Therapie der akuten nekrotisierenden Pankreatitis dar. In Japan und China gehören die „peroral endoscopic myotomy“ (POEM), die „submucosal tunneling endoscopic resection“ (STER) und die „full thickness endoscopic resection“ (FTER) als weitere NOTES-Techniken bereits zum Standardrepertoire dortiger High-end-Endoskopiezentren. Dabei stellt die enorme Patientenfrequenz auf Grund der hohen Bevölkerungszahl einen wesentlichen Innovationsfaktor dar, der dazu beigetragen hat, diese Techniken zu entwickeln und zu verfeinern. Neuerdings steht mit G-POEM („G“ für „gastric“) auch eine Myotomietechnik bei refraktärer Gastroparese zur Verfügung, deren therapeutischer Stellenwert allerdings noch nicht genau abgeschätzt werden kann. In jedem Fall hat NOTES die Endoskopie bereichert und wird vermutlich noch weitere neue Techniken hervorbringen; Japan und China werden dabei wohl erneut eine Schlüsselrolle einnehmen.
