Intracerebral hemorrhage (ICH) is the most devastating subtype of stroke. It affects approximately 2 million patients worldwide every year and is a major cause of morbidity and mortality. After decades of research there is still no specific evidence based treatment strategy for this disease. However, research has contributed to a better understanding of the pathophysiology of intracerebral hemorrhage, and has facilitated the development of different treatment approaches addressing different pathophysiologi cal mechanisms. The present article reviews some of the latest research investigating treatment strategies for ICH and intraventricular hemorrhage (IVH) and comments on the practical relevance of those data, including topics as blood pressure management, hemostasis and manage ment of oral anticoagulant-associated ICH, surgery, intraventricular fibrinolysis and lumbar drainage.

Kurzfassung: Die intrazerebrale Blutung (ICB) stellt die schwerste Schlaganfallform dar. Sie ist eine der führenden Ursachen für Morbidität und Mortalität auf neurologischem Fachgebiet und betrifft ca. 2 Millionen Patienten pro Jahr weltweit. Nach Jahrzehnten der Erforschung ist es immer noch nicht gelungen, eine spezifische evidenzbasierte Behandlungsstrategie für die ICB zu entwickeln. Das wissenschaftliche Interesse hat jedoch wesentlich zum Ver­ständ­nis der Pathophysiologie der ICB und zur entsprechenden Entwicklung therapeutischer Zugänge bei­ge­tra­gen. Die vorliegende Übersicht fokussiert auf neuere Untersuchungen dieser Therapiemethoden bei ICB und Ventrikel­blutung und behandelt die Themen Blutdruckmanagement, Hämostase und Management OAK-assoziierter ICBs, operative Behandlung, intraventrikuläre Fibrinolyse und lumbale Drainage.
