The present review summarizes the most important chronic headache syndromes, based on International Headache Society Criteria. A focus will be on chronic migraine, which may include headache days without migraine specific features. In the management of chronic migraine, it is important to recognize and treat medication overuse. An important differential diagnosis for chronic migraine is headache associated with idiopathic intracranial hypertension, particularly, if visual disturbances are present. New treatment options such as the prophylaxis of chronic migraine with monoclonal antibodies against calcitonin gene-related peptide (CGRP) will be discussed as well as neurostimulation techniques such as non-invasive vagus nerve stimulation for the treatment of cluster headache and migraine.
