The non-vitamin K antagonist oral anticoagulants (NOACs) apixaban, dabigatran, edoxaban and rivaroxaban have been shown to be simple to use, and are at least non-inferior compared to vitamin K antagonists in terms of efficacy and safety. In the present article we summarize clinical aspects regarding the correct dosing of NOACs with and without concomitant use of antiplatelet agents, in the setting of cardioversion, and perioperatively. Also, relevant interactions are presented in a concise fashion.

Kurzfassung: Die Nicht-Vitamin-K-Antagonisten oralen Antikoagulanzien (NOAKs) Apixaban, Dabigatran, Edoxaban und Rivaroxaban bieten gegenüber dem bisherigen „goldenen Standard“, den Vitamin-K-Antagonisten, Vorteile sowohl in der Anwendung, als auch hinsichtlich Sicherheit und Effektivität. In diesem Artikel werden klinische Aspekte bzgl. der korrekten Dosierung bei Patienten mit und ohne begleitende Plättchenaggregationshemmung, bei Kardioversion und perioperativ praxisnah diskutiert. Des Weiteren finden Sie eine konzise Zusammenfassung relevanter Wechselwirkungen.
