Links: Darstellung eines endokardialen Voltagen-Maps des linken Ventrikel (LV), Blick von apikal Richtung Basis/Aorta. Zu sehen ist eine elektrisch unauffällige Lateralwand des LV (violett) und eine große Narbe des interventrikulären Septums sowie Apex-umgreifend (rot) bei ischämischer Kardiomyopathie. Die roten Punkte stellen Ablationspunkte, die weißen Punkten Areale abnormer, später ventrikulärer Aktivierung (LAVAs) dar. Mitte: 3D-Rendering des präinterventionellen, hochauflösenden Herz-CTs mit intraprozeduraler Überlagerung der elektrophysiologischen Mappingpunkte aus dem CARTO3-Map. Ergänzt wurden die jeweiligen intrakardialen Ableitungen an Orten mit LAVAs bzw. Spätpotentialen. Rechts: Präinterventionelles Herz-CT mit „Late Iodine Enhancement“. Farbkodiert ist die Wanddicke des Myokards dargestellt, zudem sind anatomische Strukturen wie Koronarien, Koronarsinus und Nervus phrenicus klar zu sehen.

Das Bild zeigt ein ultra-hochauflösendes Aktivierungsmap (> 44.000 aufgezeichnete Elektrogramme) einer linksatrialen Makroreentry-Tachykardie unter Verwendung des Rhythmia HDx™-Systems bei einem Patienten mit inzessanter linksatrialer Tachykardie. Kreisende Aktivierung lässt sich um ein Narbenareal nahe der rechts-unteren Lungenvene bei bereits bestehender, elektrisch dichter Ablationslinie am Vorhofdach erkennen. Durch Ziehen einer kurzen Ablationslinie zwischen dem Narbenareal und der rechten unteren Pulmonalvene konnte die Tachykardie dauerhaft terminiert werden (rechtes Bild, rote/graue Punkte).

Electroanatomical bipolar voltage map of a left ventricle, the coronary sinus (CS) and the middle cardiac vein (MCV) of a patient with nonischemic dilated cardiomyopathy and ventricular tachycardia (VT) is shown in a caudal left posterior oblique projection. Bipolar voltage of < 0.5 mV (suggestive of a dense scar) is depicted in red and the normal myocardium (with bipolar voltage > 1.5 mV) is represented in purple, with other colors corresponding to the intermediate signal amplitude seen in the scar’s border zone.
