According to the current GOLD recommendations, bronchodilation continues to play the central role in COPD therapy, meanwhile, there is a clear indication for a supplementary application of ICS in terms of triple therapy, where necessary. The current GOLD recommendations 2020 also outline a dynamic process of stepping up or stepping down in medical treatment, and thus, the explicit possibility of de-escalation.
Non-pharmacological treatment may consist of lung volume reduction, lung transplantation, non-invasive ventilation, specialized physical therapy, pneumological rehabilitation and, of course, smoking cessation. Influenza and pneumococcal vaccination are also recommended. Adherence to therapy plays a decisive role for disease management, especially during times of COVID-19.

Kurzfassung: Im Zentrum der COPD-Therapie nach GOLD steht weiterhin die Bronchodilatation, für den gegebenenfalls notwendigen zusätzlichen Einsatz von ICS im Sinne einer Triple-Therapie gibt es inzwischen klare Indikationen. Auch ein dynamischer Prozess eines „step-up“ bzw. „step-down“ in der medikamentösen Therapie, also auch explizit die Möglichkeit einer Deeskalation, ist Gegenstand der aktuellen GOLD-Empfehlungen 2020.
Für eine nicht-medikamentöse Therapie stehen etwa Lungenvolumsreduktion, Lungentransplantation, nicht-invasive Beatmung, Atemphysiotherapie, pneumologische Rehabilitation und natürlich Raucherentwöhnung zur Verfügung. Zusätzlich werden Schutzimpfungen gegen Influenza und Pneumokokken empfohlen. Gute Kontrolle der Erkrankung durch Therapieadhärenz spielt – gerade in Zeiten von COVID-19 – bei COPD eine wesentliche Rolle.
