Pleural mesothelioma is a local­ly progressive malignant tumor of the pleura with worldwide increasing incidence and extraordinarily dismal prognosis. Systemic treatment is the standard of care. Lung-sparing surgery, intracavitary treatment and radiation treatment can be considered in selected cases, preferably within prospective trials. However, the optimal combination of these therapeutic modalities has not yet been defined properly. Since most of the present evidence comes from retrospective studies, multicentric randomized controlled trials are urgently warranted to evaluate thoroughly the real therapeutic benefit of each of these modalities.

Kurzfassung: Das maligne Pleuramesotheliom ist ein lokal wachsender bösartiger Tumor der Pleura mit weltweit steigender Inzidenz und außerordentlich schlechter Prognose. Systemische Therapie ist der Goldstandard in der Behandlung. In selektionierten Fällen können eine lungenerhaltende Resektion, intrakavitäre Therapie und Strahlentherapie erwogen werden, idealerweise im Rahmen prospektiver Studien. Die optimale Kombination dieser Modalitäten ist noch nicht ausreichend definiert. Da die etablierten therapeutischen Ansätze weitgehend auf retrospektiven ­Studien beruhen, sind dringend prospektiv randomisierte Multi­center-Studien gefordert, um den Stellenwert dieser Therapiemodalitäten eindeutig festzulegen.
