Over the past century, numerous sclerosants have been introduced, including detergents, osmotic agents and corrosive agents. Today, polidocanol and sodium tetradecyl sulfate are the most clinically relevant and best studied substances. Sclerosants may be administered in liquid or foam form. Liquid sclerotherapy is particularly suitable for telangiectasias, reticular veins and smaller varicose veins, whereas foam sclerotherapy extends the spectrum to larger tributaries and saphenous veins. The choice of substance, concentration and volume depends mainly on vein diameter, anatomical situation and treatment technique. This review summarizes the historical development, mechanisms of action, practical use and safety profile of the most important sclerosants and discusses the role of liquid and foam sclerotherapy in contemporary phlebology.
Kurzfassung: Die Sklerotherapie ist ein etabliertes minimalinvasives Verfahren zur Behandlung von Krampfadern. Im Laufe der vergangenen Jahrzehnte wurde eine Vielzahl unterschiedlicher Verödungsmittel eingesetzt, die sich im Wesentlichen in Detergentien, osmotische und korrosive Sklerosierungsmittel einteilen lassen. Heute haben insbesondere Polidocanol und Natriumtetradecylsulfat klinische Bedeutung. Die Applikation kann in flüssiger Form oder als Schaum erfolgen. Die Flüssigsklerosierung wird vor allem bei Besenreisern, retikulären und kleineren Varizen eingesetzt, während die Schaumsklerosierung das Behandlungsspektrum auf größere Seitenast- und Stammvenen erweitert. Die Auswahl von Substanz, Konzentration und Volumen richtet sich insbesondere nach Venendurchmesser, anatomischer Situation und Applikationstechnik. |
