Inpatient Video-EEG monitoring is the gold-standard for diagnosing epilepsy, for classification of seizures and of the epilepsy syndrome, for presurgical evaluation, and for assessment of seizure control.
In a retrospective study based on video-EEG monitoring in 1922 patients from the Freiburg Epilepsy center, the diagnostic yield of in-patient monitoring was assessed in patients with ascertained diagnosis of epilepsy. In 72.9 % of patients seizures could be documented in a mean recording period of 5.2 days. In a considerable subgroup of patients with low seizure frequency, monitoring did not result in the documentation of seizures. This is discussed as a relevant limitation of in-patient diagnostic work-up and in the context of newly available tools for ambulatory seizure documentation which may be helpful particularly in the monitoring of seizure frequency and seizure control.
Kurzfassung: Ein stationäres Video-EEG-Monitoring ist der Goldstandard für die Diagnosestellung
einer Epilepsie, für eine Anfallsklassifikation und
eine syndromatische Einordnung von Epilepsien, für
eine prächirurgische Evaluation und zur Dokumentation
einer erzielten Anfallskontrolle.
In einer retrospektiven Analyse basierend auf
1922 Patienten mit gesicherter Epilepsie, bei denen
am Epilepsiezentrum Freiburg ein Video-EEG-Monitoring
durchgeführt wurde, wurde der Anteil erfolgreicher
Anfallsaufzeichnungen analysiert. Bei 72,9 % der Patienten konnte bei einer mittleren Dauer des Monitorings von 5,2 Tagen ein Anfall dokumentiert werden. Dies wird diskutiert als relevante Einschränkung der diagnostischen Evaluation in einem stationären Setting und im Kontext neu verfügbarer Optionen einer ambulanten Anfallsdokumentation, die insbesondere zum Monitoring von Anfallsfrequenz und -kontrolle hilfreich sein kann.
