Aortic valve stenosis is a common cardiovascular disease especially in the elderly and is characterized by a latency period followed by an unfavourable prognosis from the onset of symptoms or the reduction of left ventricular function. Although this condition represents an indication for the replacement of the diseased valve, surgical aortic valve replacement is accompanied by an unacceptable level of perioperative risk in some patients. Transcatheter Aortic Valve Implantation (TAVI) offers an alternative treatment method for these patients, that underwent a rapid development in recent years. The following article should provide a review of the available literature and summarize current trial data.

Kurzfassung: Die Aortenklappenstenose stellt eine vor allem im fortgeschrittenen Alter verbreitete kardiovaskuläre Erkrankung dar und zeichnet sich mit einer Latenzperiode gefolgt von einer ungünstigen Prognose ab dem Auftreten erster Symptome oder der Reduktion der Linksventrikelfunktion aus. Obwohl dieser Zeitpunkt eine dringliche Indikation zur Sanierung der erkrankten Klappe darstellt, kann der konventionell- operative Klappenersatz für manche Patienten mit einem inakzeptabel hohen perioperativen Risiko einhergehen. Mit der kathetergestützten Aortenklappenimplantation (Transcatheter Aortic Valve Implantation, TAVI) steht für diese Patientengruppe ein alternatives Therapieverfahren zur Verfügung, welches in den vergangenen Jahren eine rasche Entwicklung erfahren hat. Im Folgenden soll ein Überblick über die aktuelle Literatur bzw. Datenlage gegeben werden.
