Atrial fibrillation (AF) is the most prevalent arrhythmia in adults and increases steadily in patients of higher age. AF is correlated with increased morbidity and mortality. The broad clincial specturm of AF and the consequences of anticoagulation are associated with extensive challenges for the public health systems including the outpatient departments and hospitals. In order to ensure optimal patient care a coordinated holistic and multidisciplinary approach is required.
Kurzfassung: Vorhofflimmern (VHF) ist die häufigste anhaltende Rhythmusstörung des Erwachsenen und nimmt mit steigendem Alter zu. VHF ist mit erhöhter Morbidität und Mortalität verbunden. Das breite klinische Spektrum von Vorhofflimmern und die Folgen der Antikoagulation stellen sowohl den ambulanten als auch stationären Sektor vor große Herausforderungen. Um eine optimale Betreuung der Patienten zu gewährleisten, ist ein modernes Schnittstellenmanagement mit Nutzung neuer Kommunikationkationskanäle zwischen dem niedergelassenen Bereich und den Krankenanstalten notwendig.
