If, however, fear occurs without a real threat and thus impairs the quality of life and overall functionality of a patient, it is considered pathological. Anxiety diseases are the most common psychiatric diseases and are often associated with depression and further psychosomatic and cognitive symptoms. Pathophysiologically, they are multifactorial diseases resulting from an interplay of genetic and environmental factors. Since the COVID-19 pandemic, an increase in their incidence was observed. A precise evaluation of the individual symptoms and comorbid conditions including the medical history is vital in ensuring the right diagnosis and best possible individual treatment. To enable the best possible quality of life and overall functionality, as well as to minimize the risk of chronicity, early initiation of an adequate therapy is necessary. The main pillars of the recommended treatment include psychotherapy and psychopharmacotherapy, whereby a steadily growing demand for phytopharmacotherapeutic substances was observed especially in the last decades. Therefore, promising phytotherapeutic agents have increasingly been studied in systematic international research projects. Currently, the lavender oil preparation Silexan® shows the best evidence for the treatment of anxiety diseases and depression, as well as the frequently associated sleep disturbances and psychosomatic symptoms. In mild to moderate depressive episodes, the St. John‘s wort extract WS®5570 may also be considered based on the available data. In the treatment of stress symptoms and exhaustion, positive effects have been reported for Rhodiola rosea extract SHR-5, which acts as a herbal adaptogen. Furthermore, the specialized extract EGb 761®, derived from Ginkgo biloba L., represents an evidence-based option for mild cognitive symptoms, such as in the context of Mild Cognitive Impairment (MCI). This extract has also repeatedly shown positive effects on associated neuropsychiatric symptoms, which frequently occur in the course of dementia.
Kurzfassung: Um schnelle Anpassungen in Gefahrensituationen
zu ermöglichen, ist die physiologische
Angstreaktion essenziell und evolutionär
entscheidend. Tritt die Angst hingegen ohne reale
Bedrohung auf und beeinträchtigt das Leben der
Betroffenen, wird sie als pathologisch betrachtet.
Angsterkrankungen gehören zu den häufigsten
psychiatrischen Erkrankungen und treten oft gemeinsam
mit Depressionen auf. Die Entstehung
von Angsterkrankungen und der Depression ist als
multifaktoriell anzusehen, wobei das Zusammenspiel
zwischen genetischen und Umweltfaktoren
von entscheidender Bedeutung ist. Seit der COVID-
19-Pandemie konnte eine Steigerung der Prävalenzraten
dieser Erkrankungen beobachtet werden. Um
die korrekte Diagnose stellen zu können und somit
den Patienten die optimale Behandlung anbieten zu
können, ist eine sorgfältige Anamnese und der Ausschluss
organischer Ursachen essenziell. |
