His bundle pacing (HBP) aims to pass the stimulated pacemaker impulse through the specific conduction system to the myocardium and thus achieve physiological excitation of the ventricles. The challenge here is the localization of the His bundle and the positioning or fixation of a screw electrode there. In view of the increasing evidence, the potential benefits for patients as well as the procedural and technological developments, an HBP program was established at the department in 2018, which aims to use HBP as a standard stimulation method in patients with an expected pacing rate of > 40%.
Kurzfassung: His-Bündel-Pacing (HBP) zielt darauf ab, den stimulierten Schrittmacher-Impuls durch das spezifische Reizleitungssystem auf das Myokard weiterzuleiten und damit eine physiologische Erregung der Ventrikel zu erzielen. Die Herausforderung dabei ist die Lokalisation des His-Bündels und die Positionierung bzw. Fixation einer Schraubelektrode ebendort. In Anbetracht der zunehmenden Evidenz, der potentiellen Vorteile für die Patienten sowie der prozeduralen und technologischen Entwicklungen wurde an der eigenen Abteilung 2018 ein HBP-Programm etabliert, das die Anwendung von HBP als Standard-Stimulationsmethode bei Patienten mit erwartetem Pacing-Anteil von > 40 % zum Ziel hat.
