Foley catheter is inserted into the bladder for simultaneous bladder filling and measurement of bladder pressure. From the detrusor contraction that is generated, the maximum isovolumetric pressure (Pisv), the slope of the contraction (deltaP/deltaT) and the duration that the isometric contraction remains above 50 % of its maximum (T50) can be determined. Top tracing demonstrates strong bladder contractility while bottom tracing reflects contraction that attains a similar maximum isovolumetric pressure but is phasic and poorly sustained. This patient was originally treated for OAB but failed anticholinergic treatment and other conservative measures. Videourodynamics showed no evidence of outlet obstruction but did show evidence of severe reflux into the prostatic ducts (arrows) and large pseudodiverticulum at the dome (arrowhead) indicating severe unrecognized morphologic alterations. These changes are attributed to dysfunctional (non-neuropathic) striated sphincter behavior. B) Post void film shows complete emptying of the bladder, but a substantial amount of contrast is retained in the prostatic ducts.
