Clinically the symptoms of PAD dominate, embolization and thrombosis are the most serious complications that can lead to limb loss. Indications for treatment include symptomatic aneurysms and aneurysm size > 2.0 cm. Aneurysms with thrombotic material should be treated regardless of size. Therapeutic gold standard is the open-surgical procedure (interponate, bypass) using venous material. The quality of the run off of the infragenual arteries decisively determines the openness. Alternatively, endovascular therapy may be considered under certain conditions. These option is currently not standardized because there is a lack of valid endovascular data. In addition to covered stents, „flow diverters“ are sometimes used with appropriate anatomy. Stent thrombosis, migration and stent fracture often require reintervention in the case of covered stents. In contrast, initial results of the „flow diverter“ show promising results in smaller and sacciform aneurysms.
