Its origins date back to the 1970s. The technique has been further developed through the integration of modern camera, irrigation, and drilling systems, as well as the introduction of new access trajectories such as the Kambin corridor. Transforaminal and interlaminar approaches allow for targeted decompression of neural structures. Full-endoscopic procedures in the lumbar spine demonstrate clinical outcomes comparable to or superior to open surgery for disc herniation and spinal canal stenosis – while resulting in less postoperative pain and earlier mobilisation. The greatest benefit is seen in low to moderately complex procedures, such as endoscopic treatment of lumbar disc herniations and spinal stenosis. Despite the learning curve, structured training enables safe application. Patients benefit from reduced invasiveness even in the early stages of a surgeon’s adoption. Initial investment costs are offset by shorter operating times and a favourable complication profile. Lumbar spinal endoscopy complements open procedures as a modern component of integrative spine surgery – technically, clinically, and economically.
Kurzfassung: Spinale Endoskopie hat sich in den letzten Jahren als minimal-invasive Alternative zur offenen Chirurgie entwickelt, mit dem Vorteil reduzierter zugangsbedingter Gewebeschäden und den daraus resultierenden Benefits. Die Ursprünge reichen in die Siebzigerjahre des letzten Jahrhunderts zurück. Die Technik wurde durch Integration von modernen Kamera-, Spül- und Frässystemen sowie der Einführung von neuen Zugangstrajektorien wie dem Kambin-Korridor weiterentwickelt. |
